9487254 | Sleep in rheumatoid arthritis: a comparison with healthy subjects and studies of sleep/wak | 1998 Jan | Sleep complaints are frequent in patients with rheumatoid arthritis (RA) and sleep disturbances may contribute to pain and other daytime complaints. The aims of the current study were to compare ambulatory sleep recordings from consecutively selected patients with RA to those obtained in healthy controls, and to study the relationships between sleep structure and clinical symptoms. Sleep recordings were obtained from 41 out-patients with RA and 19 matched controls. All had clinical examinations and completed different questionnaires. Recordings were scored traditionally and, moreover, the electroencephalography (EEG) was subjected to frequency analysis. For the study of sleep-wake interactions in the patients, a graphical chain model was used. The patients had many sleep-related complaints. An increase in the number of periodic movements of the legs (PML) during sleep was seen in comparison with controls, but otherwise only minor differences were observed in classical sleep stages. Data from frequency analysis showed an increase in alpha (8-12 Hz)-EEG activity in sleep stages non-rapid eye movement (NREM) 2-4 in most sleep cycles. The statistical model demonstrated a complex but independent correlation between morning stiffness, pain and joint tenderness on the one hand, and awakenings, stage NREM2, slow-wave sleep and stage REM on the other, probably reflecting a relationship between sleep patterns and pain in RA. In conclusion, only the increase in PML and alpha-EEG activity distinguished the sleep in RA patients from that of healthy controls. However, the demonstrated interaction between daytime complaints and sleep patterns may increase the understanding and treatment of the disease. In future research, graphical chain models may improve our understanding of complex relationships between multiple variables. | |
11477288 | Leg ulcers in patients with rheumatoid arthritis--a prospective study of aetiology, wound | 2001 Jul | OBJECTIVE: To study the aetiology of leg ulcers in patients with rheumatoid arthritis (RA) and to study healing and pain relief after pinch grafting. METHODS: Twenty patients with RA and leg ulcers were studied. Diagnosis of the ulcers was founded on the clinical picture and measurements of the ankle-brachial pressure index. To detect vasculitis, skin biopsies were taken for immunohistochemistry and histopathology. Pain severity was assessed pre- and post-operatively using a visual analogue scale. RESULTS: Ten of the 20 patients had ulcers with multifactorial aetiology. Fifteen had signs of venous insufficiency, 11 had histopathological evidence of vasculitis, four had reduced arterial circulation and two patients had diabetes. Healing after pinch grafting was found in eight patients, all of whom had an ulcer area less than 15 cm(2). Eleven out of 18 patients had pain reduction after pinch grafting. CONCLUSION: The causation of leg ulcers in patients with RA was found to be multifactorial, with vasculitis and venous insufficiency as the main determinants. Pinch grafting seems to be a good alternative to conservative treatment for minor leg ulcers for these patients, regarding both wound healing and pain relief. | |
9133963 | Compound heterozygosity for the shared epitope and the risk and severity of rheumatoid art | 1997 Mar | The objective was to explore the role of HLA-DRB1 genes in determining disease severity in rheumatoid arthritis (RA). The population comprised extended pedigrees of 17 multicase RA families. Family members were genotyped for both HLA-DRB1 alleles using restriction fragment length polymorphism (RFLP). Identification of HLA-DRB1*04 variants was performed using the Multiplex ARMS-RFLP technique. Compound heterozygote individuals carrying two different alleles containing the shared epitope (SE) were at greatest risk of developing RA (odds ratio = 36, 95% CI 9.1-143). A synergistic or additive effect of these alleles is suggested. Individuals carrying no SE alleles expressed milder disease, as measured by the Spread Severity (SS) index, compared to compound heterozygotes (P = 0.045). Compound heterozygosity was not invariably associated with severe disease with six (50%) having clinically mild disease at a median age of 57.5 yr and median disease duration of 16 yr. Inheriting two different SE-bearing alleles results in an increased risk of RA and, on average, greater disease severity. This is not, however, invariably associated with severe disease, making it of limited use as a predictor of prognosis. | |

A clinical study of 29 patients]. | 1999 Dec | BACKGROUND: To study the clinical features, outcome and response to therapy in 29 cases of rheumatoid arthritis (RA) with secondary amyloidosis (AA). PATIENTS AND METHOD: Twenty-nine patients with RA and AA who were diagnosed during 11 years. RESULTS: The mean age and the mean duration of RA were 63 +/- 12 years and 15 +/- 7 years respectively. The most common initial clinical feature was renal involvement (83%). Nineteen patients were treated with methotrexate. The mean survival time was 42 +/- 8 months. Eleven patients (38%) have deceased. CONCLUSION: Our data confirms that AA in RA is a serious complication with a bleak prognosis. A normal renal function is a good prognosis indicator. | |

11127682 | Femoral neck buttressing: a radiographic and histologic analysis. | 2000 Oct | OBJECTIVE: To examine the incidence, radiographic and histologic findings of medial femoral neck buttressing in a consecutive group of patients undergoing total hip arthroplasty. DESIGN: Biomechanical parameters were evaluated on standard anteroposterior pelvic radiographs of 113 patients prior to hip replacement surgery. Demographic information on all patients was reviewed and histologic evaluation was performed on specimens obtained at the time of surgery. RESULTS: The incidence of medial femoral neck buttressing was found to be 50% in a consecutive series of patients undergoing total hip arthroplasty. The incidence was slightly higher in women (56% vs. 41%). Patients with buttressing had increased neck-shaft angles and smaller femoral neck diameters than were seen in patients without buttressing. Histologic evaluation demonstrated that the buttress resulted from deposition bone by the periosteum on the femoral neck in the absence of any evidence of femoral neck fracture. CONCLUSION: It would appear that femoral neck buttressing occurs in response to increased joint reactive forces seen at the hip being transmitted through the femoral neck. The increased joint reactive force can be related to the increased neck shaft angle seen in patients with buttressing. | |

11710704 | Ability of the American College of Rheumatology 1987 criteria to predict rheumatoid arthri | 2001 Nov | OBJECTIVE: To determine how well the American College of Rheumatology (ACR; formerly, the American Rheumatism Association) 1987 classification criteria for rheumatoid arthritis (RA), when used at study inclusion in a cohort of 270 patients with early (<1 year) arthritis, predicted a diagnosis of RA 2 years later and how well they classified these patients at the end of the 2 years. METHODS: Patients were evaluated during 1995-1997 at 7 hospitals in the Brittany region of France. Patients were evaluated at 6-month intervals until November 1999. The diagnosis made by a panel of 5 rheumatologists (P5R) after the last visit was used as the "gold standard." The ACR 1987 criteria for RA were applied prospectively, without taking into account the initial diagnosis. RESULTS: At the last visit (mean +/- SD followup 29.1 +/- 11.8 months; median 30 months), the P5R diagnosed RA in 98 patients. At the last visit, classification by the ACR criteria was satisfactory, and the combination of an office-based rheumatologist's (OBR's) diagnosis of RA and fulfillment of the ACR criteria was sensitive (87%; 85 of 98 RA patients had both) and highly specific (99%; 170 of 172 non-RA patients did not have both). Application of the criteria at the first visit was of limited value for predicting a diagnosis of RA 2 years later. CONCLUSION: After a 2-year followup, the ACR 1987 classification criteria used in combination with an OBR's diagnosis were effective in distinguishing patients with and without RA. The criteria were not useful for predicting RA in patients with arthritis onset within the previous year. Some patients who met the criteria at baseline and after 2 years did not have RA, suggesting that incorporating exclusion criteria may improve the performance of the ACR criteria when used without taking into account the diagnosis by a rheumatologist, particularly in early arthritis. | |
11502074 | Cytokines in rheumatoid arthritis: is it all TNF-alpha? | 2001 Jun | Anti-TNF-alpha therapy has shown clear efficacy in the treatment of rheumatoid arthritis (RA). Since some patients do not respond and the treatment is suspensive, combination therapy may be of interest. Other cytokines produced by monocytes such as IL-1, IL-12, IL-18 are also involved. The secretion of these cytokines is regulated by subsets of T-lymphocytes. Among these, IL-17 producing Th1 cells appear to contribute directly to the destructive process. Furthermore, this T-cell contribution enhances the action of monocyte derived proinflammatory cytokines. Using models of human RA synovium inflammation and bone resorption, ex vivo results suggest that combination therapy may be of interest. Acting on more than one cytokine may increase the percentage of responding RA patients as well as the degree of individual patient response. | |
9728172 | Prediction of postoperative knee flexion in Insall-Burstein II total knee arthroplasty. | 1998 Aug | Postoperative knee flexion in patients undergoing Insall-Burstein-II total knee arthroplasty at 2 years was evaluated regarding two basic questions: what groups of patients gain or lose the most flexion and what groups of patients have the best or worst postoperative flexion. Thirteen preoperative variables (maximum flexion, flexion arc, tibiofemoral angle, quadriceps strength, extensor lag, Knee Society score, Knee Society patient assessment, gender, age, height, weight, diagnosis, and surgeon) and four postoperative variable (leg length change, tibiofemoral angle, distance from patella to the joint line, and the tibial prosthesis anteroposterior translation on a lateral radiograph) were used in an attempt to explain postoperative flexion. The analysis was performed on 164 consecutive Insall-Burstein-II total knees in which the data were gathered prospectively on a time oriented medical record database. A regression tree analysis was used to identify several groups of patients, characterized by preoperative factor values, who had markedly above average performance on postoperative flexion. The preoperative factors identified include preoperative flexion, flexion arc, tibiofemoral angle, extensor lag, diagnosis, and age. The only postoperative variable of significance was tibiofemoral angle. Among the potential determinants of postoperative flexion that failed to appear predictive were the Knee Society scores and surgeon. Preoperative flexion is known to be a critical determinant of postoperative flexion in total knee replacement. However, in the current study, preoperative flexion accounted for only half of the difference between the best (122 degrees) and the worst (88 degrees) group, as determined with regression tree analysis. | |
10668523 | Efficacy and safety of the new DMARD leflunomide: comparison to placebo and sulfasalazine | 1999 | The efficacy and safety of the novel DMARD leflunomide was compared to placebo and sulfasalazine in a randomized, double-blind study. At Week 24, leflunomide significantly reduced tender and swollen joint counts and physician and patient assessment scores compared to placebo (P < 0.001). Response rates with leflunomide were significantly greater than placebo: ACR 20% (55% vs 29%, P = 0.0001). Comparable response rates were observed with sulfasalazine (ACR 20%: 56%). Leflunomide significantly improved HAQ scores compared to placebo or sulfasalazine (P < 0.009). The onset of action with leflunomide was rapid and was seen as early as Week 2. Radiographic disease progression was significantly slower with leflunomide than placebo (P < 0.01). Leflunomide was well tolerated. No long-term safety issues were reported with leflunomide in patients who opted to continue treatment for up to 2 years. Efficacy of leflunomide in the treatment of RA was maintained at 2 years. | |
11642508 | The status of stem cell transplantation for rheumatoid arthritis: a rheumatologist's view. | 2001 Oct | Stem cell transplantation (SCT) for rheumatoid arthritis will only be appropriate for a very small proportion of patients--those with aggressive disease who have had inadequate responses to several disease modifying antirheumatic drug regimens, including tumor necrosis factor blockers. The presence of biopsy proven T cell infiltrates in the synovium may be a way to improve appropriate patient selection. While there is general agreement regarding patient selection, the specifics of these criteria are not yet delineated. As for patient selection, the most appropriate SCT regimen has not yet been agreed upon and further pilot studies in this area will be required. In contrast to the above areas in which there is a distinct lack of consensus, outcome measures are better defined in RA and should include remission rate, Disease Activity Scale or American College of Rheumatology 50 (ACR 50) and ACR 70 responses, as well as longterm benefit/risk ratios (or utilities). It would be appropriate to convene a 2-3 day conference in 1-2 years to decide on the above, after more pilot data have been developed. | |
10587553 | Peripheral ulcerative keratitis 'corneal melt' and rheumatoid arthritis: a case series. | 1999 Dec | OBJECTIVES: (1) To review the visual and systemic outcomes of patients who developed rheumatoid arthritis (RA)-associated peripheral ulcerative keratitis (PUK). (2) To describe the clinical and serological characteristics of the patients' arthropathy at the time of presentation of this rare condition. (3) To review the aetiology and management of RA-associated PUK. Patients and methods. A case series is given of all nine patients within our unit who have developed RA-associated PUK since 1996. Details of the patients' arthropathy and the serological characteristics of the RA at presentation of PUK were noted. The patients' visual outcomes and the development of any significant systemic complications were recorded. RESULTS: All patients had long-standing seropositive, erosive RA. PUK was associated with a poor visual outcome in most patients, five requiring emergency corneal surgery to prevent perforation of the globe. Two patients developed systemic vasculitis within 1 month of PUK onset, one of whom died. CONCLUSION: RA-associated PUK often has a poor visual outcome and its appearance may herald the transformation of a patient's RA into the systemic vasculitic phase. RA-associated PUK should be managed with aggressive immunosuppression if the associated morbidity and mortality are to be avoided. Cell-mediated mechanisms appear to be important in the aetiopathogenesis of PUK and a combination of corticosteroids and cyclosporin is therefore probably the regimen of choice. | |
10451064 | Health status in systemic lupus erythematosus compared to rheumatoid arthritis and healthy | 1999 Aug | OBJECTIVE: To examine health status in systemic lupus erythematosus (SLE), using both generic and arthritis-specific instruments, and compare the health status in patients with SLE to matched patients with rheumatoid arthritis (RA) and matched healthy controls. As well, to study possible correlations between health status measures and demographic and disease variables in the patients with SLE. METHODS: Patients were recruited from the Oslo county registers of patients with SLE and RA, and the healthy controls from the county population register. Measures of health status (Medical Outcome Survey SF-36, joint pain and fatigue on visual analog scale, Modified Health Assessment Questionnaire) were obtained from 82 SLE and 82 RA patients matched for age, sex, and disease duration, and from 74 age and sex matched healthy controls. Disease activity and organ damage were assessed in the patients with SLE. RESULTS: Patients with SLE were significantly more affected in all dimensions of health status compared to controls, except in the SF-36 category role-emotional. Further, patients with SLE were significantly less affected than patients with RA with regard to physical function and joint pain, but scores were similar in all other dimensions of health status. Scores of health status correlated more strongly to the damage index than to disease activity, indicating that health status measures capture some of the same concept as the damage index, namely the consequence of the disease over time. CONCLUSION: Our study showed that patients with SLE and RA have a multidimensional involvement of health status compared to healthy controls. The findings call for a biopsychosocial approach in the management of SLE. | |
9625479 | Why do we need new treatments for rheumatoid arthritis? | 1998 Apr | Rheumatoid arthritis is a chronic systemic autoimmune inflammatory disease characterized by progressive joint damage. The classical treatments of the disease such as myocrisin and sulphasalazine, are not always effective at controlling the disease. This has necessitated the development of novel agents for treating rheumatoid arthritis. Most of these drugs are biological in nature and are targeted at specific sites of the inflammatory cascade of reactions. A number of clinical trials have been conducted. The clinical effects that have been observed are transient, necessitating repeated treatments and the risk of vaccination effects. Many of these agents have to be administered parenterally, production costs are very high. Consequently, chemical entities which can be taken orally need to be developed. Since the immune system is very complex with pleiotropic cytokines and redundancy in some of the regulatory networks, it may therefore be necessary to use multiple agents targeted at different specific sites of the inflammatory cascade or that different agents could be given at different stages of the disease, to induce disease remission and maintain the response to therapy. Cytokines such as tumour necrosis factor (TNF) and interleukin 1 (IL-1) play important physiological roles in the host's defence systems against infections and malignancy. The chronic inhibition of these cytokines by targeted therapies may therefore lead to the development of side effects. Thus, carefully controlled long-term studies will be required to assess the safety of selective targeting of processes involved in inflammation. A more recent novel approach is to target hypoxic tissues with bioreductive agents. Thus, some of the established rheumatoid arthritis treatments could be linked to bioreductive agents and released in hypoxic tissues where inflammation is occurring. This review summarizes the important developments in the therapy of rheumatoid arthritis. There is no doubt that despite these developments we need to develop new and advanced treatment modalities for rheumatoid arthritis. | |

9652499 | Rheumatoid arthritis: sequences. | 1998 May | OBJECTIVE: Rheumatoid arthritis (RA) is an autoimmune disorder of unknown etiology characterized by symmetric, erosive synovitis and sometimes multisystem involvement. It affects 1% of the adult population and exhibits a chronic fluctuating course which may result in progressive joint destruction, deformity, disability and premature death. We review the literature data relative to the peculiar pathologic features of the disease shown by diagnostic imaging techniques. METHODS: All our patients were classified according to the diagnostic criteria of the American Rheumatism Association (1987). Plain radiography remains the diagnostic technique of choice, but ultrasound (US), computed tomography (CT) and magnetic resonance imaging (MRI) are also used. RESULTS: Clinically articular involvement presents as pain, swelling, stiffness and motion impairment. The patients with positive rheumatoid factor are > 70% likely to develop joint damage or erosions within 2 years of disease onset. Any joint can be involved, but the proximal interphalangeal and metacarpophalangeal joints of the hand and the wrist are preferential sites, as well as the metatarsophalangeal joint of the foot, the knee and the joints of the shoulder, the ankle and the hip. Symmetry is the hallmark of joint involvement. The synovium of bursae and tendon sheaths is also affected. Soft tissue (subcutaneous nodules), muscles (weakness and atrophy) and vessels (vasculitis) may also be involved. Systemic involvement may result in Felty's syndrome, metabolic bone disorders (i.e. osteoporosis), Sjögren syndrome and pleuropulmonary abnormalities (pleural effusion, fibrosing alveolitis, constrictive bronchiolitis). The earliest abnormalities consist in synovial proliferation, soft tissue swelling, and osteoporosis. At a slightly later stage, the inflamed synovial tissue ('pannus') extends across the cartilage surface, leading to chondral erosions and small bone erosions at the joint margin (bare areas). Marginal and central erosions follow in advanced stages and finally fibrous ankylosis, joint deformities (subluxations and dislocations), fractures and fragmentations are typical findings of more advanced RA. CONCLUSION: RA is a frequent joint disorder with a characteristic radiographic picture. Joint involvement patterns are sufficiently common to permit accurate diagnosis, especially when fusiform soft tissue swelling, regional osteoporosis, marginal and central erosions and diffuse loss of interosseous space are present. Conventional radiography remains the standard imaging technique for joint studies in the patients with suspected RA. US is recommended to diagnose soft tissue involvement (joint effusion). CT is very useful for showing abnormal processes in complex joints (sacroiliac and temporomandibular joints and craniocervical junction) which are difficult to depict completely with conventional radiography. Magnetic resonance applications include the assessment of disease activity: in particular, this technique may be the only tool differentiating synovial fluid and inflammatory pannus. | |
11171691 | Rise in serum C reactive protein after hip and knee arthroplasties in patients with rheuma | 2001 Mar | OBJECTIVE: Serum C reactive protein (CRP) concentration was evaluated in patients with rheumatoid arthritis (RA) undergoing total hip arthroplasty (THA) or total knee arthroplasty (TKA) to ascertain the postoperative CRP response. METHODS: Thirty seven consecutive patients with RA who had undergone THA or TKA were included in the study. The CRP concentration was measured in every patient once preoperatively and every other day for one week postoperatively. RESULTS: The peak median CRP concentration (94 mg/l) was achieved on the first and second day postoperatively and was seven times higher than the median preoperative concentration (13 mg/l). CRP declined to the preoperative concentration in about one week. The rise of the CRP concentration was significant (p< 0.001). No infection was encountered in this series. CONCLUSION: A rapid rise in the postoperative CRP concentration is normal in patients with RA treated by THA or TKA. The CRP concentration decreases to the preoperative value in about one week. Serial CRP measurements, including at least one preoperative measurement, are needed when the clinical significance of the postoperative CRP values is evaluated. When the postoperative CRP concentration remains raised for several days compared with the preoperative value, or even rises, it may indicate the presence of a complication in these patients. | |
10939778 | The economic evaluation of complementary medicine: a staged approach at the Royal London H | 2000 Jul | OBJECTIVE: The practical implementation of a staged, multifaceted research agenda for the economic evaluation of complementary medicine (CM) at the Royal London Homeopathic Hospital (RLHH). METHOD: The relative importance of economic evaluation as an evidence base of CM was assessed via a survey conducted with purchasers (n=481). The marginal costs of providing complementary care for patients with rheumatoid arthritis were calculated. The use, and changes in the use, of conventional medicines for patients' main complaints were established retrospectively (n=499) and prospectively (n=70). Health-related quality of life (patient utility) of newly referred patients was assessed with the EQ-5D (EuroQol) instrument (n = 70) on a 100 mm (0 = worst, 100 = best) scale. RESULTS: Economic evaluation was rated 'important' as an evidence base, after safety and RCT data ('very important'). Consultation time (doctors and dietician) contributed 29% of the total costs of treating rheumatoid arthritis. The retrospective survey showed that many patients on conventional medication were able to stop (29%) or reduce (32%) intake in the course of treatment. The median (quartiles) health state of newly referred patients was 70 mm (50,78) in men and 60 mm (36,73) in women. Some results of an interim analysis of 6 months follow-up data are reported. CONCLUSIONS: Economic evaluation of CM is becoming increasingly important and should take place by using a multifaceted, staged approach. Before embarking on randomised trials, observational data on cost, effectiveness and utility should be collected. The cost-effectiveness of CM appears to be most sensitive to the duration of the consultation. | |
9586673 | Questionnaire to evaluate the effects of assistive devices and altered working methods in | 1998 | The objectives of the study were to identify perceived difficulty and reduction of difficulty when using assistive devices and altered working methods in performing daily activities and to detect which activities were unaffected by interventions. Twenty-one women aged 29-65 years with rheumatoid arthritis answered the Evaluation of Daily Activity Questionnaire (EDAQ), which contains 102 items divided into 11 dimensions of daily activities. The women rated their perceived difficulty twice: first when not using devices or altered methods and then when using them. The use of devices or altered methods led to a reduction in perceived difficulty in 42% of the ratings. The number of items that the women found difficult when not using devices/altered methods ranged between 13 and 99. With the interventions, the number of items still found difficult decreased to between 6 and 57, 91% of the devices provided were still in use. The dimensions Eating, Cooking and Toileting contained the most items affected by the use of devices such as lever taps, springy scissors, breadknife and wrist orthosis. Few effective devices were identified for the dimensions Dressing, Washing, Cleaning and Mobility Outdoors. It was concluded that the EDAQ represents a new approach to demonstrating difficulties in performing various daily activities, to describing the effects of assistive devices/altered methods, and to identifying areas not affected by interventions. |
