Prevalence, severity and distribution between primary care and referral | 1989 | By a simple self-administered hand test, hand handicap was revealed in 689 out of a population of 5262 individuals aged 40-70 years. Subsequent medical examination showed inflammatory hand disease in 119, 67 of whom had previously been in contact with the public health service at some level because of hand dysfunction. These 67 patients formed the material for our study, whose purpose was to analyse the distribution, treatment and follow-up of patients with inflammatory joint disease on different health care levels. In primary health care, rheumatoid arthritis (RA) was found to be underreported. For half the cases the interval between onset of disease and assessment by a rheumatologist exceeded 5 years, and follow-ups were few and far between. In the referral centres, RA cases were dealt with more adequately. The prevalence of definite and probable RA was found to be 0.7 and 0.4%, respectively. | |

Results were quantified according to Helkimo's indices and compared to those in a control group of nonrheumatic adults. The prevalence was found to be statistically significant. Dental support, calculated according to Eichner, appears to play an important role. Results in the same subjects were also compared with systemic articular involvement and Ritchie's index, but did not reveal significant associations. This underlines the peculiarity of the masticatory dysfunction, its relative autonomy and multifactorial etiopathogenesis. | |
1936108 | Muscle protein synthesis in patients with rheumatoid arthritis: effect of chronic corticos | 1991 Aug | Using stable-isotope techniques, we measured rates of quadriceps muscle protein synthesis in twelve women with sero-positive rheumatoid arthritis. The results were compared to those from the normal limb of seven women with unilateral osteoarthritis of the knee. Six patients had never received corticosteroid immuno-suppression, but the other six had taken an average of 8 mg Prednisolone per day for 9 years. Quadriceps atrophy was present in both sets of patients with rheumatoid arthritis (normal legs 444 +/- 182, rheumatoid 190 +/- 40, rheumatoid + steroid 300 +/- 110 micrograms protein/micrograms DNA, means +/- SD, both P less than 0.001). Muscle protein synthesis, calculated by comparing the incorporation of 13C-leucine into biopsy samples taken after an 8 h L-[1-13C] leucine infusion with the time averaged enrichment of blood alpha-ketoisocaproate, was 0.056 +/- 0.005% h-1 in the patients not receiving steroids compared with 0.050 +/- 0.02% h-1 in normals (P greater than 0.05) indicating that muscular atrophy was primarily due to an increase in rate of muscle protein breakdown. Intra-muscular PGE2 concentration was increased in these patients (rheumatoid 0.12 +/- 0.06 ng mg-1 tissue, normals 0.06 +/- 0.03 ng mg-1 tissue, P less than 0.05). Patients taking corticosteroids had a markedly depressed rate of muscle protein synthesis (0.035 +/- 0.008% h-1, P less than 0.05) and reduced intra-muscular PGF 2 alpha concentration (P less than 0.01). We conclude that steroid therapy significantly influences the mechanism of skeletal muscle atrophy in patients with rheumatoid arthritis. | |
3579386 | Parvovirus associated antigen in the synovial membrane of patients with rheumatoid arthrit | 1987 Mar | Homogenates of synovium from patients with rheumatoid arthritis (RA) and osteoarthritis (OA) were centrifuged on caesium chloride density gradients to obtain isolates of a density similar to that of parvoviruses. Six of 11 RA isolates and none of six OA isolates reacted with an antiserum raised against a rheumatoid associated, parvovirus-like agent (RA-1 virus). An anti-B19 parvovirus antiserum did not react with any of the isolates tested. Electron microscopy of negatively stained preparations of the isolates showed that small particles of diameter 10 nm were abundant in most of the RA isolates (11/13) but absent from all OA isolates. Such particles, whose identification is unknown, were also present in RA-1 positive lysates prepared from cultured RA synovial cells. These results suggest that the RA-1 virus can be directly identified in RA synovial tissue and that the virus appears to be unrelated to the human B19 parvovirus. | |

Overall evaluation showed an excellent or satisfactory result in 15, 16, were acceptable and 10 poor. The most frequent radiological finding was upward subluxation which was present in 20 shoulders. An incomplete radiolucent zone was found around the glenoid component in 32% and around the humeral in 5%; there was no gross loosening. | |

This is the third report of reversible hepatic decompensation associated with prolonged MTX therapy in patients with rheumatoid arthritis. | |

Both groups had a significant but temporary improvement in Ritchie index. Group 1 showed a significant improvement in grip strength. No improvement was noticed in morning stiffness, 15 meter walk time and laboratory variables of disease activity in either group. Twelve patients with osteoarthritis (OA) received 2 weeks of treatment with mineral baths and mud packs. Statistically significant improvement for a period of 6 months was noticed in night pain, pain on passive motion, tenderness on palpation and in the index of severity of OA of the knee. | |

The incidence of sexual disorders was 42.3 per cent (11 patients). In this case the ejaculatory and neurohumoral components of the copulative cycle were mostly disturbed. Sexual dysfunction developed during the disease and was often associated with exacerbation of RA. Disorders in pubertal development were revealed in none of the patients. But patients with a disturbed sexual function exhibited a significant decrease in the total cumulative index in the male sexual formula, in the level of total and free testosterone, a rise in the content of prolactin. Possible causes and mechanisms of sexual disorders in male patients with RA are discussed. | |
2083305 | Reproductive factors, smoking, and the risk for rheumatoid arthritis. | 1990 Jul | Risk factors for rheumatoid arthritis were examined in a cohort of 121,700 female nurses aged 30-55, followed in the Nurses' Health Study. Baseline information on reproductive variables, cigarette smoking, obesity, and other variables was obtained in 1976 and updated every 2 years. Cases of rheumatoid arthritis were defined by standardized questionnaire and review of medical records. During 883,187 person-years of follow-up, 217 new cases of rheumatoid arthritis were identified (115 had definite rheumatoid arthritis and 102 had "undifferentiated polyarthritis"). When compared with women who experienced menarche at age 13 years, the age-adjusted relative risk of rheumatoid arthritis among women with early menarche was 1.9 (95% confidence interval, 0.9-2.4). There were, however, no significant associations between parity, age at birth of the first child, menopause, or obesity and the incidence of rheumatoid arthritis. Cigarette smokers had a slight apparent increase in the risk of rheumatoid arthritis. Among current smokers, the age-adjusted relative risk for RA rheumatoid arthritis was 1.3 (0.9-2.1); among former smokers, the relative risk was 1.5 (0.9-2.3). | |

2682774 | Soft-tissue changes in the hand in rheumatoid arthritis: evaluation with US. | 1989 Dec | High-frequency (5.0 and 7.5 MHz) ultrasonography (US) was performed in 16 patients with rheumatoid involvement of soft tissues in the hand or wrist; surgery was performed in 11. In 15 (94%) patients, a hypoechoic thickening of one or several tendon sheaths was noted. The thickening ranged from 2 to 13 mm (mean, 5.2 mm +/- 3.0). Complete or partial flexor tendon ruptures were correctly suspected preoperatively at real-time dynamic US examination in three patients. A discrete, elongated, hypoechoic soft-tissue nodule was demonstrated in five patients; one nodule was intra-tendinous. In 10 of 11 patients, US findings correlated well with those of surgery. Real-time US proved highly accurate in demonstrating rheumatoid soft-tissue changes in the hand. A hypoechoic rim around a tendon in the hand or wrist in a patient with rheumatoid arthritis is highly suggestive of rheumatoid tenosynovitis. US also provides surgeons with an accurate preoperative map of the lesions. |
