2392381 [Changes in the respiratory system of patients with rheumatoid arthritis]. 1990 Apr Rheumatoid changes in the lungs are seen in 30% of cases of rheumatoid arthritis. Three cases with different patterns of pulmonary involvement in the course of rheumatoid arthritis (RA) are presented. In all cases pleural involvement was seen, in two subjects since the beginning, in the remaining in the course of developing interstitial lung disease. In two cases the dominant changes in the respiratory system were interstitial pneumonitis. In one case the interstitial pneumonitis took a fulminating course, being resistant to treatment with prednisone, endoxan, penicillamine. In the third case, the interstitial pneumonitis was of chronic character, despite the fact that the subject did not receive any treatment, it did not progress. In the first case the main features were pulmonary cavities. The could have evolved from necrosis of rheumatoid nodules or vessel changes.

This review considers these weaknesses, and suggests criteria that should be met for the sound design of studies that are intended to demonstrate a role for personality traits and psychiatric states in the aetiology and course of the disease. These criteria include the method used to diagnose RA, sample size, the appropriateness of the tests of personality and psychiatric state for use with RA, and the matching of RA subjects with controls.

Although clinical involvement of the larynx is found in over a third of patients with severe rheumatoid arthritis, acute airways obstruction is fortunately a very rare complication. Stridor is probably precipitated in the acute situation in such patients as a result of upper respiratory tract infection.

To resolve these contradictions, the authors propose that rheumatoid synovitis represents a localized autologous mixed lymphocyte reaction.
2401118 [Magnetic resonance imaging of rheumatoid pannus in cranio-vertebral synovial joints--repo 1990 May MRI imaging can provide useful informations as to the status of the brain stem and spinal cord in patients with rheumatoid cervical spine disease. Especially MRI made it possible to visualize the soft tissue mass in the cranio-vertebral joints. However, as far as we know, there is no report describing the pathology of the soft tissue mass diagnosed by MRI. We would like to report an operated case of RA, showing that the soft tissue mass was verified as the so-called rheumatoid pannus in the field of pathology. A 67-year-old woman was admitted to our hospital because of tetraparesis and anterior limitation of neck movements. She had been suffering from rheumatoid arthritis for 17 years. On admission, her consciousness was alert. Cranial nerves were normal except for swallowing disturbance, hoarseness, weakness of neck and tongue muscles. Muscle weakness with atrophy was observed on both upper and lower extremities. Pain and touch sensations were involved below the C3 spinal levels. Laboratory examination showed mild degree of anemia, positive RA test and 40 times value of antinuclear antibody. Brain CT and cerebro-spinal fluid were normal. X-ray of cranio-vertebral joints showed erosive and sclerotic changes of the atlas and odontoid process. Metrizamide CT myelography revealed that C1 spinal cord or medulla was compressed and flattened. A soft tissue mass surrounding the odontoid process was revealed by MRI with Toshiba 0.15 tesla apparatus, showing low signal intensity on T1 image, and low and partially high signal intensity in T2 image. The first operation was done with laminectomy and posterior fusion of occipital bone and C2 spinal process.(ABSTRACT TRUNCATED AT 250 WORDS)
3487181 [Adenosine deaminase activity in levamisole treatment of patients with rheumatoid arthriti 1986 Mar 15 Nine patients who fell ill with a classical or unequivocal rheumatoid arthritis were treated with 150 mg Levamisol a week. Before the beginning of the therapy and on the day after the fourth intake of medicaments the activity of the adenosine deaminase in lymphocytes, erythrocytes and in the plasma was determined. An influence on the enzyme activity by Levamisol could not be proved. Before as well as after the Levamisol therapy the enzyme activity in the erythrocytes was diminished.

This case, as some others previously published, outlines the possibility of the association of pulmonary hemosiderosis and rheumatoid arthritis: is this a casual association, or may pulmonary hemosiderosis be a rare manifestation of rheumatoid arthritis?

The scientific organization Arbeitgemeinschaft für Rheumaorthopädie (Association for Rheumatic Orthopedics) is an official section of the Deutsche Gesellschaft für Orthopedics and Traumatology) and has very close connections with the European Rheumatoid Arthritis Surgical Society. There are many common activities within this group, especially regarding uniform documentation, which began with the elbow. Furthermore, there are standardized procedures in operative synovectomy, including new trends, e.g., arthroscopic synovectomy. Another special interest is focused on alternatives to endoprosthetic replacement, in particular on resection interposition arthroplasty. With regard to endoprosthetic replacement, the question of cementing or not is controversial; however, there are strong arguments for cementless fixation, especially for hip endoprostheses. Arthritis surgery in Germany reflects modern trends all over the world where rheumatology is a subspecialty bridging the gap between internal medicine and orthopedics.
3954457 Intracellular and extracellular sulphydryl levels in rheumatoid arthritis. 1986 Jan We detected no difference in the reduced glutathione content of erythrocytes obtained from patients with rheumatoid arthritis (RA) and controls. The stability of glutathione to oxidative stress (cumene hydroperoxide) was also the same. Although measured in the erythrocyte, our results indicate that changes in intracellular reduced glutathione are not involved in the aetiology of RA. Serum from patients with RA had a significantly reduced (p less than 0.01) sulphydryl (SH) concentration (415 +/- 89 (SD) mumol/l) compared with controls (583 +/- 74 mumol/l). This was also valid if the SH groups were expressed per gram of protein. Serum and synovial fluid from RA patients contained similar levels of SH groups (mumol/g protein).
1921478 Lymphedema of the leg associated with rheumatoid arthritis. 1991 Jun A 14-year-old boy with a two year history of seronegative rheumatoid arthritis developed left leg lymphedema and subsequently a severe episode of lymphangitis. The diagnosis of "rheumatoid lymphedema" was confirmed by lymphscintigraphy and conventional lymphography. Treatment consisted of bedrest and antibiotic drugs. When the signs of inflammation had subsided, therapy with corticosteroids was started with improvement of both joint pain and leg swelling. Whereas lymphedema associated with rheumatoid arthritis has been described in the upper limb of adults, to our knowledge this is the first report of the coexistent condition in the lower leg of a child.
3488034 alpha 1 Antitrypsin (PI) allotypes in rheumatoid arthritis. 1986 Jun alpha 1 Antitrypsin (PI) phenotypes were defined in 144 patients with rheumatoid arthritis (RA) and in 223 normal controls. The frequencies of the rare F, S, and Z variants were similar in RA and control groups. No relationships were found between PI allotypes and rheumatoid disease severity or autoantibody titre. The PI M1M2 phenotype was increased in frequency in the RA group, and phenotype frequencies in DR4 positive and negative disease were similar. These results support the suggestion that genes on the 14th chromosome which are linked to PI predispose to RA.
1953820 Agalactosyl IgG: an aid to differential diagnosis in early synovitis. 1991 Nov Sixty consecutive patients presenting with early-onset synovitis were studied by measuring rheumatoid factor (RF) titers and the percentage of oligosaccharide chains attached to the C gamma 2 domain of IgG that lack galactose (GAL[0]). After 2 years of followup, 39 patients (65%) had developed rheumatoid arthritis (RA), and 21 had developed a variety of other inflammatory joint diseases. A combination of RF positivity and GAL(0) levels above the age-corrected mean gave a positive predictive value for a diagnosis of RA in 94% of these patients. These observations may well have clinical utility.
2359069 Health status as a predictor of mortality in rheumatoid arthritis: a five-year study. 1990 May We examined the utility of health status scores as predictors of mortality in a 5-year study of patients with rheumatoid arthritis (RA). Of 279 patients with a known outcome after an average of 5 years of followup, 37 had died. Scores derived from the Arthritis Impact Measurement Scales indicated that mobility and general health perception were significant predictors of mortality with odds ratios (95% confidence interval) of: 1.19 (1.05, 1.33) and 1.27 (1.04, 1.55), respectively. These measures were particularly predictive of dying at worse levels of health status with a significant linear trend for mobility (p less than 0.05) and general health perception (p less than 0.01). Even after controlling for physical function, poorer levels of general health perception were significantly associated with mortality, (p less than 0.01 for linear trend). Our results confirm that certain health status measures are related to subsequent mortality in patients with RA.
1691798 [Quantitative analysis of serum amyloid A protein isotypes]. 1990 Feb We examined the isotypes of serum amyloid A protein (SAA), a sensitive acute phase reactant in patients with rheumatoid arthritis, rheumatoid arthritis with reactive amyloidosis and those undergoing surgery. The analysis was performed using FPLC system with Mono Q, ion exchange chromatocolumn, for the separation of low molecular fraction, which was obtained by the molecular sieve filtration. Four peaks, I-a, I-b, II and III were separated by FPLC analysis. Peaks II and III were the main isotypes of SAA and I-b was detected only in few cases. Although the recovery was low and varied after the filtration, the results obtained by FPLC were reproducible. Although peak I-a in RA with amyloidosis tended to be less than in other disorders, there was no specific patterns of SAA isotypes corresponding to each disorder.

2901567 Correlation of plasma interleukin 1 levels with disease activity in rheumatoid arthritis. 1988 Sep 24 The mean plasma level of interleukin 1 beta (IL-1 beta), measured by immunoassay, was significantly higher in 51 patients with rheumatoid arthritis (RA) than in 21 healthy controls of similar age. Further, in the RA group, plasma IL-1 beta correlated positively with Ritchie joint index, pain score, and erythrocyte sedimentation rate and correlated negatively with haemoglobin concentration. In individual patients with active disease who had serial measurements, plasma IL-1 beta also correlated with clinical disease activity. These results support the idea that IL-1 beta has a central role in the pathogenesis of RA.
2889501 Effects of sulphasalazine on faecal flora in patients with rheumatoid arthritis: a compari 1987 Oct Twenty-six out-patients with active rheumatoid arthritis (RA) were randomly allocated to treatment with sulphasalazine (SASP) or D-penicillamine (DPA). Faecal samples were collected from all patients at 4-weekly intervals and examined for changes in faecal flora during treatment. Both treatment groups showed substantial clinical improvement. In the SASP-treated group this was accompanied by significant falls in counts of Cl. perfringens and E. coli. No such changes were seen in the DPA-treated group. These results suggest that SASP's efficacy in RA may be related to its antibacterial properties.
3606369 Patient education program to teach energy conservation behaviors to patients with rheumato 1987 Jul We report a prospective, randomized pilot study comparing a new workbook-based program, designed to teach patients with rheumatoid arthritis (RA) energy conservation behaviors, with standard occupational therapy (OT). Sixteen patients took part in the new program and nine received the standard therapy. Data on the number of tender or swollen joints, grip strength, walk time, activities of daily living, psychologic adjustment to illness, and daily activity log, were measured before and three months after intervention. Eleven percent of those who received standard therapy and 50% of those who received the workbook increased their amount of physically active time (p = .10). Twenty-two percent of the control group and 50% of those in the workbook group achieved a better balance of rest and physical activity (p = .07). We conclude that the adoption of energy conservation behaviors is different in the two groups. This initial study suggests that interrupting physical activity with rest periods may result in increased physical activity in patients with RA.
3616388 Enhanced in vitro IgM rheumatoid factor synthesis in seropositive rheumatoid arthritis fam 1987 We studied the patterns of in vitro IgM RF production by peripheral blood mononuclear leukocytes (MNL) in first- and second-degree relatives and non-blood relatives of 14 RA patients. In all these included 25 individuals with RA (14 probands, 11 other affected members), 88 non-affected first- and second-degree relatives and 15 non-affected non-blood relatives. In vitro production of IgM RF and IgM by MNL from RA patients was similar to what we and others have previously described. Spontaneous production of IgM RF was demonstrated by MNL from some RA patients and the ratio of IgM RF to total IgM was higher for patients than controls. The non-blood relatives failed to demonstrate spontaneous in vitro IgM RF production. In contrast, MNL from approximately 16% of non-affected blood relatives spontaneously produced IgM RF. Spontaneous IgM RF production in this group did not correlate with HLA-DR3 or HLA-DR4 and was found in Caucasoids. American blacks, simplex (one affected individual), and multiplex (more than one affected individual) families. Linkage analysis using either a recessive or dominant model of inheritance failed to detect linkage among affected and non-affected family members.
