734376 | Kinetics of 99MTc-pertechnetate uptake in healthy and arthritic knees. | 1978 | 99mPertechnetate (99TcO-4) was injected intravenously in 7 healthy subjects and 30 patients with rheumatoid arthritis (RA). The uptake of isotope (99Tc) in both knee joint regions was traced continuously for 30 min from the time of injection. The isotope showed more rapid accumulation with increasing arthritic involvement of the knee, and the rate of uptake of 99TcO-4 in the knee region tallied with the clinical findings before and after treatment (14 RA patients). The findings indicate that the rate of 99TcO-4 uptake could serve as a useful complement to the more commonly used quantitative uptake of isotope. The two measures may possibly reflect different features of synovial inflammation. | |

Value of serum ferritin for the estimation of iron st | 1982 Dec 18 | Biochemical evaluation of iron depletion due to bleeding is difficult in rheumatoid arthritis (RA) patients with inflammatory anaemia. The authors have tried to assess the usefulness of serum ferritin (SF) level measurements--considered a reliable index of iron stores--by comparing the values obtained with true iron stores, as determined by Perls' test on bone marrow. The study involved 50 RA patients with anaemia and raised erythrocyte sedimentation rate (ESR). The poor correlation (r = 0.51) observed between the two tests in this population was mainly due to differences in ESR. SF levels and Perls' test results correlated better in patients with an ESR above 50 mm/h. (r = 0.69) than in patients with an ESR above 50 mm/h. (r = 0.44). The mean ESR values were higher than those found in patients without inflammatory syndrome. SF levels, therefore, are unreliable in patients with inflammatory syndrome, and no practical conclusion concerning iron stores can be drawn from the finding of a normal SF level. In the present study, only SF levels higher than 250 ng/ml were associated with positive Perls' tests. The behaviour of ferritin in the presence of inflammation may explain the results obtained. | |

In correspondence with the data in literature we found rheumatic nodes (70%) as most frequent extraarticular organ manifestation. In 1 patient each a participation of the lungs and a rheumatic exudate of the pleura, respectively, could be diagnosed. In another patient there were references to a vasculitis at the fundus of the eye. In only 2 patients there was a vasculitis of the digital arteries with necroses, in one of them a generalized vasculitis. 5 of the 13 neurologically examined patients showed polyneuropathic symptoms. Of the biochemical findings the C-reactive protein and the blood sedimentation rate showed clear increases and correlations. | |

Apropos of 2 cases]. | 1982 Apr | The author reports two cases of severe rheumatoid arthritis (RA) where the manifestations of necrosis vasculitis consisted of extensive and bullous gangrene of the lower limbs. In one case, the laboratory picture was that of major complement consumption (CH50 unmeasureable, marked fall in various compounds including C3 and C4) with very high levels of circulating immune complexes. An attempt at treatment by plasmapheresis, immunodepressants and corticosteroids led to moderate stabilisation of the vasculitis (regression of purpura) but remained incapable of resolving the local problem of gangrene. The severity of the lesion led to amputation of both legs. In the other case, there as a fall in CH50 with the presence of circulating immune complexes. Similar treatment led to stabilization of the lesions but final amputation could not be avoided to deal with the residue. Interruption of treatment was followed by an inflammatory exacerbation which was satisfactorily dealt with by further sessions of plasmapheresis. The authors discuss the value of such immunodepressant therapy in severe RA in the light of these cases. | |

Inoculation with 4 week- as compared to 1 week-cultured. M. butyricum raised the incidence from 34 to 63%. Inoculation of this more arthritogenic material into the F1-descendants of M. butyricum sensitive parents raised the incidence of 96%. Also the severity of the arthritis in rats selected in that manner was higher than in non-selected rats. Treatment of arthritic rats of either source with phenylbutazone resulted in a comparable dose-related reduction of the diameters of the hind-limbs. Mating of rats with various degrees of arthritic response showed that sensitivity transmission to the F1-descendants was optimal with the most sensitive parents. Although the exact nature of the transmission has not been defined, the uniform and higher sensitivity of thus selected rats is of great value for quantitative pharmacological studies. | |

The test was done when moving the neck stepwise between flexion and extension, and with the patient in both lateral and sitting positions. The test performed in the sitting position was termed the functional Queckenstedt's test. The flexed position of the neck and the sitting body position, either alone or in combination, were factors contributing to a manometric block. Six more block phenomena were recorded in the sitting compared with the lateral position. As the sensitivity of the test seemed unaffected by body position, this indicated an increased atlanto-axial luxation in the sitting position. This applied to the neutral as well as the flexed position of the neck. There was a fairly good correlation between neurological symptoms and signs and the functional Queckenstedt's test. The use of the test as an aid supplmentary to the clinical and roentgenological findings when deciding on the treatment of patients with rheumatoid atlanto-axial luxation is outlined. | |

The indication to alternative procedures has to be considered carefully for each individual case and joint. |
