7346965 Relationship between serum ferritin, anemia, and disease activity in acute and chronic rhe 1981 The assessment of anemia in patients with rheumatoid arthritis may be difficult, especially when iron deficiency and the anemia of chronic disease coexist. The development of a radioimmunoassay for serum ferritin concentration has aided the detection of reduced body iron stores in uncomplicated iron deficiency, but its use is compromised in clinically active rheumatoid arthritis by the tendency of serum ferritin to behave as an acute phase reactant. In this latter role it correlated well with disease activity in the patients we studied. Followed serially, serum ferritin levels fell in patients whose disease activity improved after institution of appropriate therapy. In anemic patients with clinically inactive disease, supplemental iron was associated with a significant rise in hemoglobin when compared to untreated patients. Serum ferritin levels behaved independently of hemoglobin levels. Therefore even in clinically inactive rheumatoid arthritis, serum ferritin does not accurately reflect an iron deficiency.
7051276 [Visceral manifestations of rheumatoid polyarthritis]. 1982 Jun 19 The commonest extra-articular symptoms of rheumatoid arthritis include nodules, vasculitis, neuro-muscular involvement and keratoconjunctivitis sicca with dry mouth, characteristic of Sjögren's syndrome. However, laboratory tests demonstrate that there is a high percentage of pericarditis and involvement of the lungs, liver and pancreas, which are clinically silent. Personal statistics on extra-articular symptoms in 110 cases of classic rheumatoid arthritis are presented.
6435244 Changes in zinc and zinc-dependent enzymes in rheumatoid patients during penicillamine tre 1984 Zinc in granulocytes, erythrocytes, serum, urine and zinc-dependent enzymes (alkaline phosphatase in serum and granulocytes, carbonic anhydrase in erythrocytes) were measured in patients with rheumatoid arthritis before and during penicillamine therapy for 6 months. The zinc concentration in serum, urine and erythrocytes increased, while granulocyte zinc decreased. Alkaline phosphatase activity in plasma remained unchanged, while alkaline phosphatase in granulocytes decreased. Carbonic anhydrase activity in erythrocytes decreased. None of the changes could be related to the activity of the disease.
3863509 Cervical spine involvement in rheumatoid arthritis. 1985 Cervical spine involvement in rheumatoid patients is common, the clinical symptoms varying from the asymptomatic state to severe occipito-cervical pain and various neurological syndromes. Atlanto-axial subluxation is the most common deformity and it should be operated on if it causes medullary compression signs or severe occipito-cervical headache and has an instability amplitude greater than 9 mm. Experience based on 155 cases of operatively treated rheumatoid cervical spine instabilities and deformities is described and discussed.
6415865 Association of HLA antigens, toxic reactions and therapeutic response to auranofin and aur 1983 Sep To investigate the possible relation between HLA antigens and either favourable clinical response or toxic reaction to two different gold compounds, aurothioglucose and auranofin, a new orally absorbable gold compound, we studied 50 patients with rheumatoid arthritis prospectively. Of the 25 aurothioglucose-treated patients, response to treatment could not be evaluated in four patients because of early toxicity. Nine patients showed an excellent response, while 12 were moderate or non-responders. Four of the 9 excellent responders were HLA-DR3 positive, but none of the 12 moderate or non-responders possessed this antigen (P less than 0.025). In accordance with previous reports HLA-DR3 was found more frequently in patients developing toxicity on aurothioglucose than in those who did not (RR = 5.6). No association was found between HLA antigens and favourable clinical response in 25 auranofin-treated patients. Two of them showed a severe adverse reaction, neither of them being DR3-positive. HLA-DR3 positivity is associated not only with drug toxicity, but also with excellent respondership to aurothioglucose treatment.

Lack of association wit 1978 Jul In independent studies, 51% and 36% of patients with rheumatoid arthritis have been shown to possess HLA-Dw4 compared to 7% and 13% of controls. In one study Dw4 positive patients more frequently had rheumatoid factor in their sera than did Dw4 negative patients. In order to determine if the Dw4 specificity is associated with the disease or with the presence of rheumatoid factor, the frequency of this HLA antigen has been determined in 24 healthy women known to have rheumatoid factor (median titer 1:160). Only 3 were found to have the Dw4 specificity, suggesting that this specificity is not associated with rheumatoid factor in the absence of rheumatoid arthritis.

A study of 89 patients (author's transl)]. 1981 Jan 25 Chronic interstitial pneumonia, necrobiotic nodules and pleural lesions are felt to be extraarticular manifestations of rheumatoid arthritis (RA). In a prospective study, posteroanterior and lateral chest X-ray films were taken in 89 patients with classical or definite RA (15 male and 74 female), of whom 64 (71.9%) were seropositive. The findings were compared with those of 103 age and sex matched controls. The chest X-rays were blindly evaluated by two of the authors (a pneumologist and a radiologist), and the presence of an interstitial pattern, nodules or pleural lesions was recorded. The following results were obtained: Interstitial pattern, in patients with RA, 5/89 (5.6%), and in controls, 5/103 (4.6%). Pulmonary nodules, in RA, 2/89 (2.2%), and in controls, 2/103 (1.9%). Pleural adhesions, in RA, 32/89 (35.9%), and in controls, 24/103 (23.5%). The results of the present series demonstrate that radiological pulmonary lesions are not more frequent in RA than in controls, similar conclusions having been reached by other authors.
4059884 A comparative study of pain description, emotional discomfort and health perception in pat 1985 The purpose of the present study was to compare pain description and emotional and psychosomatic discomfort in patients with chronic pain syndrome (CPS) and rheumatoid arthritis (RA) in order to identify special characteristic features in patients with CPS. Methods used were a Visual Analogue Scale, the McGill Pain Questionnaire, a new pain, ache and hurt assessment tool and a questionnaire related to causes of pain, health perception and experienced emotional and psychosomatic discomfort. The patients with CPS perceived themselves as being less healthy than patients with RA. While the sensory quality of pain was similar in both groups, the emotional component was stronger in patients with CPS. They experienced more emotional and psychosomatic discomfort compared to patients with RA. They believed that factors associated with their work had caused the pain. The result indicates that assessment of ache by patients with CPS on the Visual Analogue Scale reflects the affective dimension of the experience.
4062388 MacIntosh arthroplasty for the rheumatoid knee: a 10-year follow up. 1985 Nov The results of 75 MacIntosh arthroplasties performed for rheumatoid arthritis of the knee in 63 patients were reviewed at least 10 years after surgery. Forty-two knees in 35 patients were available for assessment. Eleven arthroplasties had been revised to total knee replacement without difficulty because of pain or poor function. The remaining 31 knees in 25 patients gave good or excellent results in 22 cases, fair in eight, and poor in one. Seven patients could not be traced, and 21 patients representing 26 knees had died. At least half these knees had given satisfactory results immediately before death judged by review of the case notes. The difficulty of comparing functional status with the preoperative state because of progressive multiarticular disease was highlighted. Although greater angular deformities preoperatively reduced the chance of success in the medium term, late failure of the arthroplasty after five years was very rare. Approximately two-thirds of all the arthroplasties performed gave satisfactory results at 10-year follow up or until the time of death.
6520831 Rheumatoid arthritis: the effects of treatment with dapsone on hemoglobin. 1984 Dec Hemoglobin concentration (Hb) was measured at 6 week intervals for up to a year in 84 patients with rheumatoid arthritis treated with dapsone. During the first 6 weeks, mean Hb decreased from 12.0 to 11.0 g/dl (p less than 0.001). Falls in Hb occurred in 81% of patients but exceeded 2 g/dl in only 10%. After more than 6 weeks of treatment mean Hb increased, though a few individuals became anemic during this period. Anemia led to discontinuation of dapsone in 5 (6%) patients. Men and patients receiving corticosteroids had higher pretreatment Hb values; after falls in mean Hb at 6 weeks, recovery was more rapid in these 2 subgroups. Changes in Hb produced by 100 mg and 150 mg dapsone daily were similar.
6273769 Neuromuscular transmission in rheumatoid arthritis, with and without penicillamine treatme 1981 Dec Electroneuromyographic studies were performed on patients with rheumatoid arthritis, some of whom were received penicillamine, to determine whether a subclinical defect of neuromuscular transmission existed. There were no significant differences between patients and controls with respect to nerve conduction studies or repetitive ulnar nerve stimulation. Four patients (three receiving penicillamine) demonstrated mild neurogenic changes distally on needle electromyography. Mean jitter was slightly higher for patients receiving penicillamine than in other patients or controls, but the differences were not significant. No significant correlations existed between of the studies and daily, cumulative, or average penicillamine dosage. A significant positive correlation (p less than 0.001) existed between jitter and duration of disease in patients receiving penicillamine. Results were consistent with the hypothesis that penicillamine predisposes certain individuals to develop myasthenia gravis rather than interfering directly with neuromuscular transmission.
7422501 [Hereditary deficiency in complement C7 and platelet aggregation disorders associated with 1980 Aug 30 In a patient suffering from rheumatoid arthritis thorough investigations led to the discovery of complete deficiency in the 7th component of the complement. A study of the patient's family tree confirmed the hereditary, codominant autosomal character of the deficiency, unrelated to the HLA system. The complement-dependent serum functions were investigated: the opsonizing and chemotactic activities were preserved, but bactericidal properties were lacking. Coagulation studies showed disorders in platelet aggregation in the presence of thrombin, and these were corrected by the addition of C7. The association of a dysimmune disease with C7 deficiency is probably not fortuitous; it might result from a common gene abnormality or, in some cases, from repeated infections.
6102877 Sulphasalazine in rheumatoid arthritis. 1980 Feb 16 Seventy-four patients with rheumatoid arthritis were treated with sulphasalazine. There was a significant improvement in clinical score, with substantial falls in serum C-reactive protein concentrations and erythrocyte sedimentation rate four weeks after starting the drug. Improvement was maintained in the 38 patients who remained on the drug for one year. The mean Rose-Waaler titre did not change. There was little difference between the results in seropositive and seronegative patients. The commonest adverse effect was dyspepsia, but five patients developed a megaloblastic anaemia and one patient neutropenia; all made a complete recovery. The results suggest that the drug has a disease-modifying action not attributable to its "salicylate" content. The mode of action might be by an antibacterial effect on gut flora.

A report of three cases. 1985 Feb Three women with well established rheumatoid arthritis (RA) who developed pulmonary hypertension during the course of their disease are reported. In one patient there was mild pulmonary fibrosis but this appeared unrelated to the severity of the pulmonary hypertension. Another patient had increased platelet stickiness which may have contributed to this complication, whereas in the third patient, no contributing factors were apparent. The pulmonary complications of RA and their relationship to the development of pulmonary hypertension are discussed.
6395813 The timing of prednisolone dosage and its effect on morning stiffness in rheumatoid arthri 1984 Dec Forty-one patients with rheumatoid arthritis (RA) maintained on low dose prednisolone (mean 5.8 mg) participated in a double-blind cross-over study to determine the effect of timing (morning or night) of prednisolone dosage on morning stiffness. Prednisolone given at night resulted in a significantly shorter duration of morning stiffness (p = 0.0001) than did an equivalent dose given in the morning.
7310140 A new radioimmunoassay for IgM and IgG rheumatoid factors, based on a double antibody meth 1981 A new radioimmunoassay has been developed for measuring IgM and IgG rheumatoid factors. Diluted sera from donors and patients were incubated with immunoprecipitates prepared from sheep serum and rabbit anti-sheep IgG antiserum. The precipitates were washed, and radioiodinated rabbit F(ab')2 fragments specific from human IgM or IgG were added. The precipitates were isolated from filtration and measured in a gamma counter. With this assay IgM rheumatoid factors were detected in sera from all patients with seropositive rheumatoid arthritis and in sera from 40% of patients with seronegative rheumatoid arthritis. IgG rheumatoid factors were found in sera from 68% of the seropositive and 40% of the seronegative patients. Gel filtration experiments demonstrated that it is possible to detect monomeric IgG rheumatoid factors and IgM rheumatoid factors of molecular weight smaller than pentameric IgM. Furthermore it has been shown that IgG rheumatoid factor activity is still present after reduction of IgM rheumatoid factors with dithiothreitol.
7175842 Ocular pathology, tear production, and tear lysozyme in rheumatoid arthritis. 1982 Sep Ninety-seven rheumatoid arthritis (RA) patients receiving investigational nonsteroidal antiinflammatory drugs were found to have a 31.9% prevalence and 24.5%/year incidence of ocular pathology. None of these ocular lesions were definitely related to their RA or drug therapy. Twenty-one of these patients were studied further for evidence of keratoconjunctivitis sicca. None of these patients had abnormal rose bengal staining, but over half had decreased tear production on both stimulated and unstimulated Schirmer's testing and many patients had high tear lysozyme levels. Increased tear lysozyme levels and decreased Schirmer's tests correlated with a modified rheumatoid activity index.
7364831 Articular changes associated with a flexion deformity in rheumatoid and osteoarthritic kne 1980 May In a consecutive series of 87 operations for knee replacement prospective observations were made of the pattern of articular destruction in the form of a triangle of erosion or osteophyte formation, which is seen on the medial femoral condyle, sometimes with an associated strip on the lateral femoral condyle. These areas represent the surfaces which are normally in contact with the anterior horns of the menisci when the knee is fully extended. The lesions in question were found only in association with a flexion contracture in 84 out of the 87 cases.
831768 Unexplained pulmonary hypertension with pulmonary arteritis in rheumatoid disease. 1977 Jan The case is described of a 24-year-old woman who died after suffering from rheumatoid arthritis for eight years. During the last four months of her life there was clinical evidence of pulmonary hypertension. At necropsy the heart showed right ventricular hypertrophy but no congenital defect. Microscopic examination of the lungs disclosed evidence of hypertensive pulmonary vascular disease with medial hypertrophy and intimal fibrosis of muscular pulmonary arteries together with hypertensive changes in the pulmonary arterioles. Some of the muscular pulmonary arteries also showed fibrinoid necrosis of their media and acute arteritis. It was concluded that the fibrinoid necrosis was a minifestation of severe hypertensive pulmonary vascular disease rather than being a primary rheumatoid arteritis of the lung.
